PATIENT CENTER MEDICAL HOME

Welcome to our practice!  We would like to introduce ourselves, our services, and our policies:

Your New Medical Home

In our effort to provide high quality patient centered care, we are honored to be recognized as a Patient Centered Medical Home (PCMH) through the National Committee for Quality Assurance. As your medical home, Clover fork Clinic, provides accessible, continuous, comprehensive, patient-centered, coordinated, compassionate, and culturally effective care. We are committed to coordinating your treatment through your primary care provider and/or care management team to ensure you receive the most up-to-date, evidence-based care.

Working in a team-based approach, we offer whole-person care, evidence-based preventive and wellness care for diabetes, and other chronic conditions, care when you are sick and education and self-management support.

REGISTRATION

During your first visit to our office, registration information will be collected.  This information and any other information given to our office will be kept in the strictest confidence.  At each visit, you will be asked to present your insurance card and photo identification.  Additionally you will need to verify:

  • Address

  • Phone Number

  • Insurance

  • Preferred Pharmacy

 

This is not an all-inclusive listing of each identifier captured and verified in the registration process.  Please be prepared to provide additional information beyond the examples listed above.

Payment

At check-in, your account will be reviewed for any outstanding balances, and any relevant co-payments.  Payment in full is expected at the time of service.  You can pay by check, credit/debit card, or cash.  Please make checks payable to Clover Fork Clinic.  You will receive a receipt for each paid office visit.  Charges for laboratory tests performed by our reference laboratory (outside lab) will be billed to you by the reference laboratory.

 

If you are unable to provide payment at the time of service, please make arrangements with our staff BEFORE leaving the office.  A statement will be mailed at the end of the month.  We expect prompt payment.  We realize that special situations arise that requires alternative payment plans.  In these circumstances, we offer individualized, structured payment plans, as well as Family Assistance.  Please discuss these with our staff before your account is overdue.

If your insurance is not listed above, please ask receptionist if it is accepted.

 

If you are on Medicare or Medicaid, please notify our office staff when you arrive for each appointment.  We need a copy of your Medicare/Medicaid card at each visit.  If you fail to provide us with this information, you will be responsible for any fees incurred during your visit.We accept Medicare assignment.  Our staff is available to answer any questions.

Hours of Operation

Our office is open Monday through Friday from 8 a.m. until 4:30 p.m in Evarts. Our Harlan Office is open Mondays 8:30 a.m. until 7:00 p.m. and 8:30 a.m. until 5:00 p.m. Tuesdays through Fridays.  Our Evarts Pharmacy is open 9:00 a.m. until 5:30 p.m. Mondays, Wednesdays, Thursdays and Fridays.  The pharmacy hours are 10:00 a.m. until 5:30 p.m. on Tuesdays. WE OFFER FREE PHARMACY DELIVERY.  On-call staff is available 24 hours a day and 7 days a week, see additional details below.

Days Off

If your primary care provider is out of the office, you may be scheduled with one of our physician assistants or another primary physician.  Our healthcare providers collaborate daily, as part of a care team, and are fully prepared to manage each patient’s individual care needs.  It is important to become familiar not only with your primary care provider, but also with each health care provider on staff at The Clover Fork Clinic.

 

Appointments

Appointments are preferred.  You can arrange an appointment at our Evarts office by calling (606) 837-2108 and choosing option “0” which allows you to speak with our helpful staff. You may also call our Harlan office at (606)573-1975.  They can help you schedule or cancel an appointment and also assist with any other need or request. For any non-urgent visits, including acute care/sick needs, we encourage you to log in to your Patient Portal to request an appointment.  To effectively manage your care, at each visit please inform your healthcare provider of all medications you are taking, any drug allergies, or side effects, visits to other health care clinics, such as specialist visits or hospitalizations, changes in your medical history or health status, results on any outside lab or diagnostic testing, and updates on your self-care needs.

Pediatric Transition

Clover Fork Clinic accepts pediatric patients transitioning to adult health care.  We will assist the patient and/or family to identify a provider, transfer records from a pediatric setting and communicate with the office regarding any special needs of the patient.  This should be done near the patient’s 18th or 21st birthday or sooner if the patient is an emancipated minor.

Well Child Visits

For your convenience, we recommend that you request appointments for physical examinations and routine checkups at least 2-4 weeks in advance.  Other appointments are scheduled as available and necessary.  Walk-ins and call-in appointments are available as needed.

Cancellations

If you are unable to keep a scheduled appointment, we ask that you notify our office as much in advance as possible.  Early cancellation allows someone else to use that appointment time and you to reschedule as soon as possible. 

 

If you arrive more than 10 minutes late for an appointment, you may be asked to reschedule or to wait until other scheduled patients have been seen.  This avoids long waits for you and other patients, and provides adequate time for a thorough visit.

After Hours

The Clover Fork Clinic maintains an after-hours and weekend nurse line.  The nurse line provides non-emergency advice, access to your medical record and can contact your primary provider if necessary.  The nurse line number is (606) 505-0462.  If you, or your family, need emergency care after hours, please dial 911 and/or contact Harlan Appalachian Regional Healthcare at (606) 573-8100.

Telephone Calls

When you call our Evarts office, our automated attendant will give you options.  We encourage you to choose appropriately so that your call can be routed efficiently.  The options are as follows:

  • Press 2 – Scheduling

  • Press 3 – Medical Records

  • Press 1 – Pharmacy

  • Press 4 – Billing/Insurance

 

Calls regarding laboratory tests or x-ray results, when done at another facility will not be returned until results are available.  Any abnormal results will be reported to you as soon as the physician has reviewed them.  These results can be reviewed at your next appointment.  If you have activated your Patient Portal account, these results may also be posted to your account for your review.

Medication Refills and Samples

Contact your pharmacy when you are low on medication.  Your pharmacist will check to see if you have any remaining refills on your prescription.  If you do not, then your pharmacist will contact us for approval.  If you are calling our office regarding medication, please make sure you have the medication available and can read the label information directly from the bottle.  While medication refills should always be initiated by the pharmacy, we understand there may be special circumstances that arise.  If you need to contact us directly for your refill, please do so while you have at least 2-3 days’ worth of medication remaining.

Your Personal Physician

The relationship between you and your physician, and the care team (clinic staff) is the driving force behind a Patient-Centered Medical Home.  Your physician will provide medical care that is right for you based on clinical guidelines shown to improve health.

 

Your Care Team

Your physician or physician assistant will direct the care team to coordinate your care based on your wants and needs.  To improve efficiency, the team will plan for your appointment by reviewing your medical chart for up-to-date forms, check for recent testing, and ensure you are notified of results in a timely manner.  The care team will work with you and any outside providers/facilitates to effectively coordinate your healthcare across all specialties.  Your care team will make sure your appointment is timely, provide the specialists with your care summary before your appointment, and obtain the specialists recommendations after your appointment.  If you are admitted to the hospital, you will not receive care from a Clover Fork Clinic physician or physician assistant.  However, our providers, who are familiar with your health history, will review your health record and familiarize themselves with your current condition.  Upon discharge, our care team will review your hospital stay, make sure you return for follow-up care, and discuss any questions or concerns you may have about your treatment of medications.

 

Your Health

In return, we ask that you continue to be an active participant in your health care.  We ask you to take charge of your health by responsibly managing and monitoring aspects of your care.  Keep us informed of your medications, visits to specialists, medical history, health status, recent test results, self-care abilities, and information from recent hospitalizations, emergency or urgent care visits.  In addition, you can help by being prepared and on time for your appointment and bringing all of your medication bottles along with you.  Follow the care plan your physician has developed for you, and let your physician know when you can’t.  We are here when you need us; you can call our office, or follow the guidelines for reaching us during non-business hours for advice before going to the Emergency Room in a non-emergent situation.

Quality for you

Our PCMH is committed to providing same day appointments and offering expanded hours to meet your needs.  We will use our electronic medical record, Practice Partner, to support the best care, quality, and safety by helping us to identify and provide for your needs and the needs of our entire patients population.  Additionally, we are able to contact you via electronic reminders for appointments, preventative, and chronic care services.

Physicians

Dr. Sharon Colton began caring for people at the Clover Fork Clinic as a practicing physician in August 1994.  She is board certified in family medicine and received her medical education from the University Of Louisville School Of Medicine in 1991.  Dr. Colton interned and completed her residency at The University Of Missouri-Columbia in 1992-1994.  Additionally, Dr. Colton earned a Bachelor of Science in Medical Technology from Marquette University in Milwaukee, WI (1973) and Physics, Genetics and Immunology at Alice Lloyd College in Pippa Passes, KY in 1987.  She is a member of the Harlan County Medical Society, American Medical Association, American Academy of Medical Acupuncture, and the Kentucky Medical Association.  Dr. Colton has also been teaching as a preceptor for programs from Yale University, George Washington University, University of Kentucky, and University of Louisville.  She continues to see patients at our Evarts and Harlan locations as well as Harlan Health and Rehabilitation Center and The Laurels, an assisted living facility in Harlan County.  Dr. Colton currently practices at our Evarts location on Mondays, Thursdays, and Fridays and our Harlan location on Tuesdays and Wednesdays.

Dr. Kristen Nash, DO, began caring for people at the Clover Fork Clinic as a practicing physician in July 2020.  She is board certified in family medicine and received her medical education from the University of New England College of Osteopathic Medicine (UNECOM) in 2017.

Dr. Nash interned and completed her Family Medicine Residency at Guthrie Clinic/Robert Packer Hospital, Sayre, Pennsylvania July 2017-July 2020.  Additionally, Dr. Nash earned a Master of Public Health in Epidemiology from the University of Alabama at Birmingham (UAB-SOPH) in 2013 and a Bachelor of Science in Medical Biology from the University of New England in 2011.  Dr. Nash was licensed by the Commonwealth of Kentucky August 2020.  She also has been active staff of Harlan Appalachian Regional Hospital since 2020 as well as family practice teaching staff at Lincoln Memorial University (LMU). 

She is a member of the American Academy of Family Practice, American Medical Association, and the Kentucky Academy of Family Practice.  Dr. Nash has also been clinical teaching staff while working at the Clover Fork Outpatient Medical Project, INC.  This includes teaching and leadership in a preceptor program for Yale University, James Madison University, and the University of Kentucky.  Dr. Nash also continues to see patients at the Harlan Health and Rehabilitation Center.  She currently practices at our Evarts location Mondays, Tuesdays and Thursdays and our Harlan Clinic on Wednesdays and Fridays.

Dr. Mia R. Sellars, D.M.D., graduated from the University of Louisville School of Dentistry in May 1993. She then completed a two-year Advanced Education in General Dentistry residency program from July 1993-May 1995. She was then employed by the University of Tennessee-Memphis Dental Operative Department. At the same time she was employed by UT-Memphis, she was employed as a general dentist in a private office in Memphis, Tennessee.  Dr. Sellars was employed by the Clover Fork Outpatient Medical Project INC. in August 1996, and has been back practicing in her home county for the past 27 plus years.  She has provided oversight and care of Clover Fork Clinic’s school-based dental program in the Harlan County School System as well as the clinical practice at the Evarts Clinic.

Physician Assistants

Mike Napier, Physician Assistant Certified, began caring for people at the Clover Fork Clinic as a Physician Assistant in 1979.  He completed the Clinical Associate Program for Physician’s Assistants, University Of Kentucky in 1979 as well.  Additionally, he received a Bachelor of Science Degree in Biology from Union College, Barbourville, KY in 1977.  Mike is a member of the American Academy of Physician’s Assistants and the Kentucky Association of Physician’s Assistants, where he has served as Vice President 1984-1985.  He has served since 1982, and continues to serve, as a Clinical Preceptor, for Senior Physician’s Assistant Students, for Yale University, University of Kentucky, James Madison University and others.  Mike was honored with the Physician Assistant Clinical Rotation Award from Yale University in 1991 and named the Physician Assistant Preceptor of the Year by the University of Kentucky in 1992.  He currently practices at our Evarts location Mondays, Tuesdays, and Wednesdays and our Harlan location on Thursdays and Fridays.

Will Miller, Physician Assistant Certified, began caring for people at the Clover Fork Clinic as a Physician Assistant in August of 2009.  He completed his Bachelor of Health Science, Physician Assistant Studies at the University of Kentucky in 1997 as well as receiving a Master of Science in Physiology in 1999.  Will received his Bachelor of Science in Psychobiology in 1992 from Centre College.  He currently serves as a Board Member for the Harlan Independent Schools, President of the Harlan County Historical Network and Parish Council Member at Holy Trinity Church.  Additionally, he was the 2013 Committee Chairman for Boy Scout Troop 149.  Will was honored as the Harlan County Health Professional of the Year in 2007.  Will currently practices at our Harlan location on Mondays, Wednesdays, and Fridays and our Evarts location on Tuesdays and Thursdays.

Eric Roberts, Physician Assistant Certified, began caring for people at the Clover Fork Clinic as a Physician Assistant in October 2013.  He completed the Physician Assistant Program at the University of Kentucky in 2013 and had previously attended Alice Lloyd College where he received a Bachelor of Science Degree in Biology in 2009.  Eric is a member of the Kentucky Association of Physician Assistants (KAPA).  He was honored with the Eileen Van Dyke Rural Service Award, by the University of Kentucky Physician Assistant Program, in 2013.  Eric currently practices at our Harlan location on Mondays, Tuesday, and Thursdays and our Evarts location on Wednesdays and Fridays.

Nurse Practitioner

Jessica Turner, Nurse Practitioner APRN, began caring for people at the Clover Fork Clinic as a Nurse Practitioner in January 2024.  She completed her Master of Science in Nursing at Chamberlain University in August 2020 and had previously obtained her Family Nurse Practitioner Specialty Track, Bachelor of Science in Nursing – Chamberlain University, in March 2019.  In addition, Jessica received her Associate of Science in Nursing from Southeast Kentucky Community & Technical College in May 2016.  Jessica is Family Nurse Practitioner Certified (FNP-C) by the American Academy of Nurse Practitioners (AANP).  She currently practices at our Harlan location on Mondays, Tuesday, and Thursdays and our Evarts location on Wednesdays and Fridays.
